If you re-run the test, and also while the board is still cold, do you consistently get a failure for TX_2 -> LNA_W loopback?

have the same with my Lime SDR mini. Except, in my case sometimes test failed and sometimes not. When it failed it is always at the tx2/lnaw loop test.
But…that was with the recent PothosSDR-2020.01.26-vc14-x64.
This morning i have downloaded a version PothosSDR-2019.01.26-vc14-x64, and programmed again the lime sdr mini with the lime suite gui.
When i look in lime suite gui i see at device info that the "gateware"version was 1.3 and now is 1.28.
Now i started again the lme sdr quick test again and again and maybe 30 times: ALL TESTS PASSED.
So do not send back your Lime SDR mini but first try this. Maybe a bug in the recent software???

Thanks, this is a useful data point.

@Zack, @Garmus, any thoughts on this?

Well…i am not sure if it is useful anymore since i had again some loop test failed. Even with 1.28.
I know the test has to be done with no cables or equipment connected and a few days ago i saw in another topic that you have to do the test when the Lime is cold.

I have done some more tests now just after start-up and then the test is passed.
Anyway, when the test is NOT passed…the Lime is still working perfect in transmit as far a i can tell.
So, i stop testing and continue in normal use.
